Move the “Insert” button on the Insert/Edit Image window to the right. As you can see in this screenshot, every other window you’re dealing with has the Confirm Action button on the bottom right, which makes perfect sense, but when you go to confirm the insert of an image, most will accidentally be clicking the “Cancel” button instead because they are used to confirming whatever the desired action is in that spot.
