if a previous revision of a content item is promoted to be the current revision, this information is not stored in the audit trail. Anyone aware if there a reason for this, and is there any way of making it happen?

It is, effectively, you just have to ‘interpret’ the audit trail. After a promotion, the audit trail has entry whose revision is equal to the revision you promoted from. This rev # will be smaller than the latest revision in the list.
